SUMMARY
Objetive: To assess the impact of pharmaceutical care both on pharmaceutical cost reduction and the decrease in hospital admissions of polymedicated patients with chronic disease in a Health Care Center.
Materials and method: Quasi experimental study conducted with two groups of patients, each one from two different Health Care Centers. Only one of the groups received intervention, and the other one was used as a control group. Both groups included patients treated with 7 or more medicines during five months previously to pharmaceutical intervention between November 1st, 2012 and March 31st, 2013. Pharmaceutical care was conducted from November 1st 2013 to March 21st 2014, period in which pharmaceutical cost and hospital admissions directly related with these patients was evaluated.
Results: 283 pharmaceutical interventions were carried out on the 84 patients from the first group, of which 225 were accepted (82,41%). 178 medicines were suspended (23,54%).
The pharmaceutical cost during these five months was decreased in 16.770,94 € (31,68%), and hospital admissions also decreased in the intervention group (56,25%).
Conclusions: Pharmaceutical care for polymedicated chronic patients has been proven to reduce the number of inadequate drugs as well as the number of hospital admissions. Appropriate medication and its modification under clinical security criteria are the main reasons for the decrease in hospital admissions.
